HIPAA Privacy Practices

HIPAA is a set of federal laws and regulations
governing, among other things, the privacy of health related
information. One
of the requirements is that we notify you of how we use and handle
your health information in what is called a Notice of Privacy
Practices. You also must sign a form stating that you have received
a copy of this notice (you can simply read a copy online or in
our office). You only need to do this once.

You can read, download, and/or print our
Notice here on our website. It is available in both ” and ” formats.
On the last page of the document is the form that you need to
sign and bring to the office with you so that we have it on file.
You can also print this document separately as a ” file,
sign it, and bring it with you to your visit.
